How do I get services?
For individuals with private insurance please contact your insurance provider, and contact us through the CONTACT link above and we will assist in the process. Be sure and complete the intake form HERE.
For individuals on the Medicaid Waiver (CES/SLS or HCBS) contact your case manager/ resource coordinator at your local CCB and request our services. Complete the intake form HERE.
Individuals receiving a traumatic brain injury grant contact your case manager at Denver Options, and complete the intake form HERE.
Children 0-3 in need of ABA services should contact their child find representative, and complete the intake form HERE.
BSOTR is a Program Approved Service Agency (PASA) for the following Community Center Boards:
